RECOMMENDED USAGE

Adults 12 years and older: Take four level scoops (53 g) of Replace® and add gradually to 200 ml of water or milk - while stirring (mix until powder is dissolved). Makes 7 servings.

Maltodextrin, sucrose, vegetable fat powder (palm oil), milk protein concentrate (cow's milk), soy protein isolate (soya bean), vegetable fat powder (Sunflower oil), inulin, vegetable fat powder (linseed oil), magnesium amino acid chelate, flavouring, calcium lactate, potassium citrate, potassium chloride, calcium carbonate, choline bitartrate, sodium chloride, sodium citrate, vitamin and mineral premix (vitamin A palmitate, cholecalciferol, DL-alpha-tocopheryl acetate, menaquinone, thiamine hydrochloride, riboflavin, pyridoxine hydrochloride, cyanocobalamin, folic acid, biotin, nicotinamide, pantothenic acid, vitamin C, chromium picolinate, potassium iodide, ferrous fumerate, manganese sulphate, sodium molybdate, zinc oxide, selenium) and calcium phosphate tribasic.

STORAGE

Keep this product in a cool (under 25 °C), dry (below 65 % humidity) and ventilated place to extend shelf life. Store away from direct sunlight. After opening and resealing this product, it should be used within one month. Do not refrigerate.

If pregnant, nursing or taking medication, consult your healthcare practitioner before use. 

Do not use this product if you have an allergy or sensitivity to any of the ingredients.

Tube feeding: Administer tube feeding as directed by your healthcare practitioner. The condition and physical ability of the patient to cope with formula will dictate the pace and quantity of administration, as well as the strength of the dilution. Prepare and administer Replace under hygienic conditions.
